On the slot-loader, the hard drive is normally set to master with the optical drive being slave on the IDE bus. If the drive has distinct settings for +master (single)+ and +master (with slave drive)+, use the latter. The +cable select+ setting will not work.
To install Mac OS 9, you need to have a Mac OS 9 installation disc. I would find one that is at least 9.1. According to everymac.com, the first 'Indigo' model (400 MHz) came with 9.1 and so did the second (500 MHz).

Macs can generally boot the OS version it came with or later, but not earlier, so a disc with 9.0 or 9.0.4 probably will not work. 9.2.1 should work.
The disc should also be a 'retail' disc (the ones that came in a shrink-wrapped box). They are white with a big orange '9' on it. If you get a disc that came with a specific Mac model, unless that Mac model is the model you have, it may not work.
If you decide to use Mac OS X with it (it can go up to 10.4.11 with sufficient RAM), and it has never previously run Mac OS X 10.2 or higher, be sure the 4.1.9 firmware update has been installed BEFORE attempting to run the installer.
